CleftPALS
Qld Inc is a voluntary group comprising parents, professionals and other
individuals interested in the welfare of individuals born with a cleft
lip and/or palate and their families.
The Federal Government’s
National Privacy Principles which are set out in the Commonwealth
Privacy Act 1988, represent a legal obligation which we are required to fulfil.
We are committed to the
protection of your privacy by acting in accordance with the requirements
of the Privacy Act and the national Privacy Principles.

website at www.privacy.gov.au
We
treat all information with care to ensure confidentiality is maintained
at all times, is accurate and protected from misuse.

What
information do we collect and what is it used for
The
amount and type of information we hold about you depends on the nature
of your relationship with us and the extent of service you require.
To
enable us to provide a high level of service to you, we ask that you
provide certain information to us including your name, address and
contact details including telephone and fax numbers and email addresses,
personal and medical details of the cleft affected person, their medical
history and details of any ongoing treatment.
We
will only collect your personal information where it is necessary for
one or more of the functions or activities we perform when providing a
service to you.
Examples
of these services include membership, providing newsletters (including a
welcome to new babies) and other documentation, specialist feeding
equipment, assistance with pre-natal and pre-op and post-op feeding and
care, preparation for surgery, matching with appropriate Contact Parent
and families where required, and programs we run such as Birthday Club.
Information on the cleft condition is also collected for
statistical purposes and while this information is made public, it is
done so in a non-identifying manner.
All
the information we collect assists us to respond to the needs of the
cleft community including planning and improving existing services and
also in developing and identifying other products or services that may
benefit or interest you.
Other
ways we may use your information include assisting individuals or
institutions such as hospitals and universities who are conducting
research, trials or surveys relating to clefts. However,
consent will always be sought from you prior to disclosing any personal
details for this purpose.
Your Rights
You
are entitled to ask what information about you is being collected and how,
and for what purpose it will be used.
You are also entitled to check your information and request changes be
made if you believe it is inaccurate.
How do we protect your information?
CleftPALS
Queensland Inc is not permitted to provide anyone with your personal info
unless you personally give your consent.
CleftPALS
Queensland Inc takes reasonable steps to ensure the personal information
kept about you is secure, accurate, complete and up-to-date, and is
handled in an ethical and lawful manner.
How to access your information
Depending
on the nature of your request, we may require you to put your request in
writing to the President at the address listed on the back of this
brochure.
Should
you submit a request to access your personal information, we will release
to you details of the information we hold which we are required by law in
accordance with the National Privacy Principles
Complaint
You
can lodge a complaint with us regarding any breach of our privacy
obligations to you by using any of the contact methods at the back of this
brochure.
You
may also contact us regarding any issues or concerns you have regarding
the way in which we use your personal information.
We are committed to addressing your concerns promptly and
appropriately.
